Our experienced staff includes registered nurses, physical therapists, occupational therapists, speech therapists, medical social workers, home health aides, dietitians, wound care specialists and diabetes specialists. Under the direction of each patient’s personal physician, registered nurses or other allied professionals with diverse clinical experience will be coordinated to provide specialized in-home care. Lake Forest Hospital Home Health Care is licensed in Lake, Northern Cook and McHenry counties and is accredited by The Joint Commission. Home health care enables men with chronic needs or those recovering after hospitalization to heal in the comfort and convenience of their own home.
